Das kommt vielleicht ganz drauf an, was du als Speichermanager lädst, insbesondere wenn es sich dabei nicht um HIMEM.SYS handelt.
Laut Abschnitt 1.5 des
DOS/4GW Configuration Guide deaktiviert z. B. der Extender die A20-Leitung beim Wechsel vom Protected Mode in den Real Mode, außer wenn er Software wie XMS-Speichermanager erkennt, die diese benötigen. Man kann diese aber auch aktiv halten:

Außerdem enthält das Dokument nützliche Infos, wie man z. B. den virtuellen Speicher von Anwendungen um eine Auslagerungsdatei erweitern kann.
In DOS/32A funktionieren diese Umgebungsvariablen-Optionen leider nicht.
@Blue: Ich würde dennoch mal eine minimale (F5-)Konfiguration ohne alles, also auch Soundkarten-Treiber (Worms sollte dann schon mal, wenn auch ohne Soundausgabe laufen), probieren.
